A Patient Coordinator will guide each candidate along the way to ensure all questions and concerns are answered by Dr. Nikko and his staff. Once a breast surgery plan has been selected, the Patient Coordinator will review the pricing details, financing options and surgery dates available.
- Pre-op: all breast augmentation patients are required to conduct a Mammogram X-ray prior to the procedure. Two weeks before the surgery date, Dr. Nikko will review all the details of the before, during and aftercare with the breast surgery patient. During this two-hour visit, candidates have the opportunity to address all questions and concerns and share their final goals to ensure their look is achieved.
- Surgery: surgeries are conducted at Nikko’s ambulatory surgery center located inside the Nikko Cosmetic Surgery Center. All breast augmentation or breast surgeries are performed under general anesthesia and generally last about two hours. Dr. Nikko, along with his experienced staff that includes qualified nurses and CRNAs, will conduct the surgical procedure to ensure all the proper measures are taken. Once the patient is awake from the general anesthesia, Dr. Nikko will prepare them for discharge and to return the next day. All patients receive a free meal courtesy of Nikko Cosmetic Surgery Center to alleviate any extra work for the caretaker.
- Post-op: Dr. Nikko requests to see all patients 24 hours following a breast augmentation or breast procedure. During this appointment, he will ensure the patient and the breasts are healing adequately. Also, the nurses will remove any bandaging and clean the incision sites to prevent infections. The patient will again visit Dr. Nikko six days proceeding surgery to remove sutures and avoid any visible scaring in the incision sites. All patients visit Dr. Nikko numerous times for up to a year to ensure the results are beyond expectations. The post-op visits are free of charge; thus included in the package.
What factors do I consider for a breast augmentation with Dr. Nikko?

What are the incision site options?
Plastic surgeons have their own preference when choosing an incision site to place an implant for breast augmentation. There are four ways to insert the implants and all include advantages and disadvantages.
- Transaxillary: Dr. Nikko’s number one and main choice, the transaxillary incision site has many advantages when inserting an implant. When placing the incision under the arm, the scar is hidden beneath the armpit crease, avoiding any visible scars on the breast. Another mayor advantage of conducting a breast augmentation using the transaxillary incision site is it allows the implant to be placed above or below the muscle.
- Transumbilical: a fiber-optic camera is tunneled into the belly button to a pocket under the breast through which the implant is inserted into the breast and is then inflated with saline. This incision site is possible only with sub-glandular implant placement and with saline implants. Although Dr. Nikko has performed numerous breast augmentations via the belly button, he believes this scarless incision site does not offer many advantages to a natural breast look.
- Inframammary: this incision is made under the breast fold and even though the scar is not as noticeable as the nipple incision, it’s still visible. Since the incision bypasses the milk ducts, the women typically experience less difficulty breastfeeding and allows for placement of the implants both under the muscle and the glandular tissue.
- Periareolar incision: Dr. Nikko rarely recommends since this incision leaves a visible scar around the areola and obstructs the milk conduits for breast feeding. Also, most women with this type of breast augmentation incision experience problems with nipple sensation following the surgery.
What implant placement is the most popular?
Current breast size, amount of breast tissue, body type and exercise habits are some of the factors Dr. Nikko must consider when deciding on the implant placement. However, as one of the most renowned Cosmetic Surgeon’s in Houston, Dr. Nikko’s preference lays on the subfascial placement.
Subfascial: this placement is located under the fascia (connective tissue covering the muscle) and above your pectoralis muscle. It allows placement of implants below a thin firm layer of connective tissue covering the muscle. Dr. Nikko prefers this method over the submuscular because it allows to position without lifting the muscle attachments from the ribs. Also, this position does not push the implant down and out as does the muscle in submuscular position, creating a more natural cleavage. The healing process and rippling is also more controlled with subfascial placement. According to Dr. Nikko’s experience, the subfascial position provides less chance of capsular contraction than any other placement.
- Submuscular: although not a preference for Dr. Nikko, in some cases the placement of the implant allows a more natural look behind the pectoralis muscle. This placement does not obstruct mammography. The recovery from this procedure is longer and the patient might experience muscle post-op discomfort. In addition, the implants take longer to settle until the muscle relaxes. There are two types of submuscular positions:
Partical submuscular: under the pectoralis major muscle and partially covered by muscle thus the implants are mostly behind the breast. While this approach may lead to minimal capsule contracture and rippling, and unobstructed mammography, flexing muscles can contract implants into unnatural appearance. In addition, since the bottom of the implants is supported only by the skin tissues, the risk of downward bottoming-out is greater than total submuscular position.
Total submuscular: under the pectoral and non-pectoral muscles and totally covered by muscles thus the implants are totally behind the breast. This type of implant placement can only be achieved by transaxillary approach. The advantages of this approach are minimal capsular contracture and very low mammography interference. In addition, there is no implant visibility which leads to even more natural look.
Subglandular: Dr. Nikko very seldom will recommend this placement of under the breast tissue but above the pectoral muscles. The subglandular breast implant placement may result in a quicker recovery since only skin and fat are cut and the muscle is not affected. However, the major drawbacks are the risk of capsular contracture or visible rippling because the implant is covered only by skin or tissue. In addition, this breast implant placement obstructs mammography more than the other techniques.
How do I choose a Breast Implant? Saline vs. Silicone
Breast implants are artificial medical devices that enlarge, shape and enhance the breasts. They come in variety of shapes, surface textures and sizes and all breast implants have a shell and a filling material. The shell is usually made of silicone elastomer with the exception of polypropylene implants which are silicon free. When choosing an implant, Dr. Nikko will also recommend the best fit based on each patient’s goals. There are two choices for breast implants:
- Saline: silicone bags filled with sterile saltwater solution. These implants can be either pre-filled or filled after being placed and are usually selected to achieve a perkier, rounder look. The incision made for breast augmentation surgery is much smaller since the breast implant is filled after the surgery is complete. Saline breast implants are available in different shapes and textures:
- Contoured (anatomical): shaped like a teardrop – some women prefer this type of breast implants because they mimic the natural shape of the breast. The breast pocket is carefully made to prevent an implant from rotating thus distort the shape of the breast. The shell of these breast implants is textured to decrease the chance of capsular contracture
- Traditional (round): shaped like a globe – some women prefer this type of breast implants because they can rotate within the breast pocket without distorting the shape of the breast. The shell of these implants is smooth so that the breast implant can freely move within breast pocket.
- Silicone: silicone bags filled with a silicone, cohesive memory gel. Even though these implants were pulled off the market for health concerns, research studies show that silicone implants are as safe as saline-filled implants. Silicone gel has a consistency that closely resembles that of natural breast tissue, so silicone breast implants are often undetectable after they have been placed. Also, the viscosity of the silicone gel makes the rippling that sometimes occurs with saline breast implants much less common. Because of this, women who are very thin or who need reconstructive breast surgery achieve much better results with silicone breast implants. The size of the incision made for breast augmentation surgery is larger to accommodate the size of the pre-filled implant.
